Prepare for Your Enrollment
Taking time to sit down and make benefits decisions each year is often the last thing you want to do. At the same time, it may be one of the most important decisions you will make all year.
To make the process easier, follow these steps:
- List any medical, dental, or vision procedures you have planned for next year.
- Determine the amount you spent on prescriptions last year. You may be able to log in to your pharmacy’s website to find all of your information.
- Look at your budget! Work to understand the gap between your deductible and how much you can afford for out-of-pocket costs such as copays, prescriptions, and unexpected medical bills.

Dependent Verification
To enroll dependents, you will need to prove eligibility. Below is a list of documents you can use to verify dependents.
- Current federal income tax return
- Marriage license (if your spouse participates)
- Birth certificate for children including adopted and stepchildren
- Proof of disability if a child with a disability over the age of 26 participates
If you are covering a dependent child over the age of 26, you’ll need to provide your most current federal income tax return naming the child as a claimed dependent. You’ll also need proof of total and permanent disability which existed before the child turned 26 or while the child was a full-time student.
