Student Records Requests
Transcripts and other records may be submitted online through our Student Records Request form. Please submit your records request with accurate information. Inaccurate information could delay your request. No orders are taken over the phone, and walk-in orders will not be processed if not previously submitted online. Proof of ID must be uploaded for each request. All requests are processed within 3-5 business days. Records may be faxed to, mailed to, or picked up at the District Office by appointment only for the requesting party.
A processing fee will apply. See the processing fees for commonly requested records below:
- $2.00 per Transcript
- $5.00 per Individual Education Plan (IEP)
- $15.00 per Cumulative Record copy (currently enrolled students only)
Cash or money orders payable to Azusa Unified School District are accepted by mail. When sending your payment please include the name of the student for whom the records request is for.
