Emergency Cards
It is extremely important that correct information is on file in the school office for each student. Please let the school office know when there are changes in address, home telephone, business telephone, or persons to be contacted in an emergency situation.

Illness & Injuries
Any student who becomes ill or injured during the day should report to the health office or main office. A staff member will contact the parents/guardians to determine if a release from school is recommended. Any serious injury at school may result in the paramedics being called. Having family or school medical insurance is highly recommended.

Medications
Any medications needed by a student during the school day must be kept in the office. These medications must be brought to school in the original container. An Azusa Unified School District “Physician’s Recommendations for Medication” form must be completed by the physician and signed by the parent in order to have medication administered at school. Students may not carry medication (including aspirin, cough drops and cold remedies) at school. Students may carry asthma inhalers and epipens if it is certified that they are knowledgeable, capable, and responsible enough to use them appropriately.
